O-desmethyltramadol is an active agonist of the μ-opioid receptors and has a lower affinity for δ- and κ-opioid receptors, its action is associated with G-proteins.
(+)- the two tramdol enantiomers show different pharmacological exposure profiles, both of them are inactive as serotonin reuptake inhibitors, but (-) – the enantiomer remains active and acts as a norepinephrine reuptake inhibitor. Its effects could be useful in the treatment of pain, where there is a likelihood of interactions compared to other opioids.
O-desmethyltramadol is a partial antagonist of the serotonin 5-HT2C receptor, where inhibition of the receptor is a factor in the antidepressant effects of agomelatin and maprotilin. It causes the release of dopamine and norepinephrine, which regulates mood and anxiety.
